呼出气一氧化氮(FeNO)在支气管哮喘管理中的价值

摘要:目的 评估呼出气一氧化氮(FeNO)在支气管哮喘管理中的价值。方法 选取未规范治疗的哮喘患者30例,给予布地奈德福莫特罗粉吸入剂治疗8周,检测治疗前后的FeNO、肺功能、ACT评分、血清总IgE及诱导痰。同期选取30例健康志愿者,检测FeNO。结果 哮喘组治疗后FeNO水平较治疗前显著降低(34.50±11.85 vs.72.93±21.88 P<0.01),治疗前后的FeNO水平均显著高于健康对照组(72.93±21.88 vs.9.87±2.84;34.50±11.85 vs.9.87±2.84,P=0.000),治疗后的FEV1%pred及ACT评分均显著高于治疗前(73.34±6.83 vs.71.45±7.83; 22.70±2.00 vs.19.83±3.12, P=0.000),治疗后血清总IgE、诱导痰Eos%均显著低于治疗前(154.17±37.55 vs.203.17±51.20;7.22±2.07 vs.9.56±2.33,P=0.000)。治疗前后的FeNO水平与FEV1%pred、ACT评分呈显著负相关(r值分别为-0.931,-0.973,P<0.01),与血总IgE、诱导痰Eos%呈显著正相关(r值分别为0.766,0.756,P<0.01)。结论 FeNO是一种评估气道炎症的可行性指标,有助于评估哮喘的控制情况及指导治疗。
